Pay on arrival form

Use this form to request that supplements be set aside for pickup and will be paid for upon arrival. When your items are ready for pickup, you will receive a text; come at any time during our open hours to pick up and pay for these supplements (Monday - Thursday until 5:15 pm, and Fridays until 1 pm). 

Please attach pictures of your supplements to the form under "Supplement Photo Upload." These pictures help us gather the correct kinds/variations/dosages of your supplement. 

If not picked up in 2 days, your items will be returned to inventory and will need to be ordered again. A quicker alternative would be to pre-pay for your supplements, as described here.

Since our team members are constantly helping our clinicians with appointments and operational logistics, they are not always available to assist walk-in requests.  You might have to wait if you have not made arrangements to have items set-aside (on hold).
