To help those clients who want to drop by the office to pick up their supplements, but choose to pay for them upon arrival, we offer the option to use the form below. This is like a quick text, except you can take a quick picture of your supplements and attach it when you fill out the form. The picture really helps us out to get the correct items for you. (e.g. we have more than 10 different kinds of "vitamin B12", multiple variations of CoQ10).
NOTE: items requested to be set aside will be held for pickup (M - Thur, until 5:15 pm, and Fridays until 1 pm when the office closes). If not picked up in 2 days, they will be returned to inventory. You will have to complete another request at a later time. An alternative, quicker way, is to use the Pre-Pay option. Securely order and pay for your products, and select "pick up" as an option. PAID orders are held until they are picked up.
Since our team members are constantly helping our clinicians with appointments and operational logistics, they are not always available to assist walk-in requests. You might have to wait if you have not made arrangements to have items set-aside (on hold). Fill out this form before you come into office for supplement pickup.
